Being Scientific in Incident Investigations

Incident investigations can be very challenging and require an objective methodology to determine the contribution causes. The revised CCPS investigation book, NFPA 921 and other references include the Scientific Method to provide a systematic approach that includes testing of hypotheses. The Scientific Method is age old and easily understood and practiced. Yet, investigation teams often rely on judgements to justify their final hypothesis without objectively testing their hypothesis. Unfortunately, they may not have found the real contributing causes. The danger is that corrective actions will not address the real root causes. The presentation will describe and demonstrate the use of the Scientific Method.
